- The General Practitioner (GP) records inc Treatment Plans
- Full historical psychiatric / psychology records including diagnosis
- Medication History
- Court Case Bundle &/Or legal Documents including any previous capacity assessments
- Reports & Records from other Practitioners not featured in GP records (e.g. Psychologist/Psychiatrists/ Nurses/Speech & Language Therapist)
- Social Service Records (including any Statement of Special Educational Needs and related assessments)
- Residential Home records & reports
- Educational Records
- Copies of witness statements
- Copies of family statements
- Any family &/or advocate statements
- Any other applicable records relating to the individual
